Botox is a bacterial toxin produced from the bacterium Clostridium botulinum. This toxin acts on the neuromuscular junction, creating a temporary paralysis in the muscles. It is used in the treatment of wrinkles caused by dynamic facial movements. Although it is the most frequently performed aesthetic procedure, its cosmetic use has become widespread in the last 20 years.

As of 2025, there are 4 legally approved products in Turkey: Dysport, Botox, Nabota, and Bocouture. The product Masport enters the country illegally and can cause many problems.

Botox temporarily paralyzes the muscles where it is injected. It does not completely paralyze the muscle; it affects about 1 cm around the injection site. Its effect is never permanent, and once it wears off, the muscles return completely to their original state.

If the wrinkles are caused by facial expressions, Botox will remove them. However, if the wrinkles are present even at rest without expressions, they are static wrinkles and Botox may not eliminate them. In such cases, regular Botox sessions or combining it with fillers may be necessary.

The effect of Botox reaches its maximum in the 2nd week. By the 3rd month, the muscle effect decreases by 50%, and it completely disappears between 4–6 months.

Botox applications are usually repeated every 4–6 months. With repeated applications, the intervals may become longer.

Which areas of the face are treated with Botox? Botox is most commonly applied to the upper face: around the eyes, between the eyebrows, and on the forehead. In addition, it can be applied to the mouth area, nose, and neck.

• Eye corners: Reduces the wrinkles known as “crow’s feet,” slightly lifts the outer corners of the eyebrows, and improves upper eyelid heaviness, giving a more refreshed look.
• Between the eyebrows: Prevents or weakens frowning. Reduces vertical “anger lines” and slightly lifts the inner part of the eyebrows. • Forehead: Reduces horizontal forehead lines by relaxing the muscles, creating a smoother appearance.
• Lip lines: Reduces vertical lines around the lips (especially known as “smoker’s lines”).
• Lip corners: Lifts the corners of the mouth slightly and softens the downward “sad lines.”
• Nasal tip: Lifts the nasal tip slightly when injected between the nose and upper lip.
• Bunny lines: Reduces fine lines on the bridge and sides of the nose that appear when smiling.
• Chin: Reduces dimples and unevenness on the chin area.
• Mona Lisa Lift: Tightens sagging skin along the jawline, giving a more lifted lower face.
• Neck: Relaxes the vertical neck bands (“turkey neck”), creating a smoother and more elegant neck line.

In aesthetic surgery, Botox is also used for teeth clenching (masseter treatment), excessive sweating, and migraine therapy.

Botox is highly effective for excessive sweating in the underarms, palms, and soles. The effect lasts about 6 months on average. When applied once a year, usually in spring, it provides comfort during summer months.

Underarm applications are particularly effective, as the sweat glands in this area are odor-producing glands. There is no proven harm; the toxins are eliminated naturally from other parts of the body.

For some migraine patients, Botox is very effective. When injected into the trigger points of migraine pain, it can reduce or completely eliminate the frequency and intensity of the headaches.
Teeth clenching treatment – Masseter Botox
For patients with masseter muscle hypertrophy and jaw pain caused by clenching or grinding, Botox injections relax the muscle and reduce tension.

There is no proven harm or poisoning effect of original, medically approved Botox products used for aesthetic purposes.

Before the procedure, an anesthetic cream is applied. After waiting about 20 minutes, the muscles to be treated are marked, and Botox is injected. The procedure is not painful – in my experience, it is even easier than eyebrow plucking.

The procedure takes about 10 minutes.

What should be done before Botox? It is recommended to avoid blood-thinning agents such as aspirin or omega-3 supplements before treatment, although they are not an absolute contraindication.

The head should not be tilted downward for about 12 hours.
Avoid hot environments such as saunas or Turkish baths for one week.
Avoid wearing tight goggles for one week.
Do not wash the face for the first 6 hours; if necessary, use cold water only.
Avoid sun exposure for the first 4 days.
Avoid facial massage and skincare treatments for one week.
Energy-based treatments such as radiofrequency, microneedling, or PRP should not be done after Botox, as they can reduce its effect. These should be performed before the injection.

Mild bruising and swelling may occur at the injection sites and resolve in a few days
. Some patients may experience temporary headaches in the first week, which can be relieved with painkillers.
One of the significant side effects is drooping of the upper eyelid (ptosis), which occurs if Botox is injected too close to the eyelid muscles. In this case, special eye drops can be used until the effect wears off. Sauna sessions and skin treatments like PRP or mesotherapy can help speed up recovery.
If performed incorrectly, Botox can create an unnatural look. This can be avoided by consulting an experienced plastic surgeon.
